We ask all teams to fill following application form
We prefer to obtain all documents by e-mail (info@ffch.cz), which is more useful for the following preparation of the championship. Our secretary Jitka Bočková will collect all documents and she will contact you in case that something would missing. After we will obtain all documents, we will mark it in the table in page „Teams“.
Hotel offers except double and single rooms also 28 apartments composed from two bedrooms and one bathroom, we prepared also price for this apartments (for cases that there will stay either 2 persons or 3-4 persons).

Please, write to the e-mail which participants would like to share rooms, if not, we will distribute you in the order you will record the names in the application form.
Every member of team and person who would like to fish have to get Czech state rod licence
1) We ask all participants to:
– print application forms (.pdf or .jpg), – we apologize, but we don´t have full electronic version
– fill full name, address, date of birth, number of passport or ID card
– sign it.

If you come sooner for practice before the championship and inform us about the place of your stay, we will deliver rod licences for you to this hotel.
If not, we will deliver all licences to Wellness Hotel Frymburk by May 19th 2014 and you can pick them up there.
We ask all teams to send all documents and to make payment by March 31st 2014, later registration will be not accepted!!!

Regarding correct completion of the Fishing Permit applications:
– We have no ID cards in NZ – can we leave this field blank?
– Can we apply for rod licences from 19/05/2014 (on the same form)?
– On the Fishing Permit form: what do we put in the „in/on“ fields?

– instead ID card fill passport number
– rod licences are valid from date of issuing, it will be April, so no problem
– fill only big rectangles on the top

Can I please have some assistance with the following banking issues….
1. IBAN NUMBER has a slash/ between the numbers 6/6 in the numerical sequence. My bank is asking if we can remove this / when we organise Interntional payment.
2.The bank also needs a beneficiary name, What is the beneficiary name please? I assume it is the first 2 lines of the address but is never good to assume.. Can you please confirm asap. Perhaps amend your PDF.
